Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to coordinate long-term services for a defined caseload, traveling to client sites across multiple counties in Florida. This field-based role requires residency within commuting distance and willingness to travel.

You will conduct assessments, develop PCSPs, coordinate with Medical Directors and interdisciplinary teams, and document member goals.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Education and experience may be required based on state law and contract requirements.

Responsibilities

• Perform face-to-face program assessments using predefined questions.
• Coordinate and manage a member's LTSS/IDD and BH/PH needs with a care team.
• Support development of a Person Centered Plan (PCSP) and backup plan.
• Interface with Medical Directors and participate in interdisciplinary rounds.
• Document member goals with their care team and circle of support.
• Identify members who may benefit from alternative services or other waiver programs.
• Mentor or precept new staff and contribute to process improvements.
• Submit utilization/authorization requests with care plan alignment.
• Report critical incidents to state and county agencies and handle audits.
