Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing individualized treatment plans, delivering therapy, and linking them to essential services. Qualifications

• Bachelor’s degree in a human services discipline required.
• Minimum of two years experience with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapeutic interventions as outlined in the plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the plan.
• Serve as liaison with professionals, agencies, and community resources.
• Maintain accurate documentation of client interactions and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support when required by the program.
• Ensure compliance with state regulations and COA standards.
• Participate in ongoing training and professional development.

Education

Bachelor’s degree in a human services discipline
