Job summary

Health Connect America seeks a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to essential services. You will collaborate with a treatment team, recruit referrals, and ensure meticulous documentation while upholding COA standards.

The role requires a Bachelor’s degree in a human services field and at least two years’ experience with children and/or families.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships and volunteer work allowed).

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions, as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
