Job summary

Health Connect America is seeking a dedicated Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. This role requires effective communication, regulatory compliance, thorough documentation, and active referral recruitment.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families.
• Provide individual, group, and family therapeutic interventions as outlined in the treatment plan.
• Attend weekly Treatment Team meetings and participate actively.
• Link clients to services and resources identified in the treatment plan.
• Maintain accurate documentation of client interactions and progress notes.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
