Job summary

Affinity Health Management is seeking a hospice aide for the night shift to support patients under RN supervision. Responsibilities include personal care, safety, documenting observations, and assisting with mobility and daily tasks in a compassionate, confidential manner. Requires CNA completion, CPR certification, and COVID vaccination.

Prior hospice or long-term care experience preferred. Must be 18+, have a valid driver’s license, and basic EMR skills.

Qualifications

Completion of a state-approved CNA course or equivalent training.
Current Healthcare Provider CPR certification.
Due to facilities with multiple covid guidelines, must be covid vaccinated.
High school diploma or GED.
Ability to read and follow written instructions and document care given.
Comfortable providing specialized care to the terminally ill and hospice philosophy understood.

Responsibilities

Perform personal care tasks and attend to patient requests promptly.
Communicate with patients and families with empathy and respect.
Ensure safety and proper use of equipment (foot stools, side rails, O2).
Provide baths, grooming, oral hygiene, shampoos, and linen care.
Assist with dressing, feeding, and basic mobility as directed.
Assist with medications reminders as allowed by state regulations.
Maintain clean, safe patient environment and proper documentation in EMR.
Report changes in patient condition to RN Case Manager and care team.
Participate in quality improvement activities and maintain professional standards.
