Job summary

Affinity Health Management is seeking a hospice aide for the night shift (5pm–10pm) to support patient comfort under RN supervision. The role involves reporting observations, maintaining documentation, and delivering compassionate personal care in a hospice setting.

The ideal candidate will be a state-certified nursing assistant with CPR certification, able to read and follow care plans, and committed to patient dignity and confidentiality. Covid vaccination per facility policy is required.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on, not internet-based).
• Covid vaccination as per facility guidelines due to working with multiple facilities.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and can provide specialized care to the terminally ill.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care tasks after assignment with prompt attention to requests.
• Communicate with patient and family respectfully, maintaining confidentiality.
• Ensure safety by using equipment correctly (foot stools, side rails, O2, etc.).
• Provide personal care: baths, back rubs, oral hygiene, shampoos, changing linens.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding; avoid homemade foods prepared by the caregiver.
• Monitor skin care to prevent pressure ulcers.
• Report pain or changes to RN Case Manager promptly.
• Assist in ambulation and exercise as instructed by nurse or therapist.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regs (reminders).
• Assist with bedpans, urinals, light housekeeping, laundry.
• Perform ROM and simple procedures with competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep patient living area clean and orderly.
• Follow organization care documentation procedures and professional conduct.
• Participate in quality assessment/ performance improvement activities.
• Respect patient and family environment and personal needs.
• As mandatory reporter, report failures to comply with state requirements to authorities.
