Job summary

Affinity Hospice Management LLC is seeking a dedicated Hospice Aide for a 5:00 PM to 10:00 PM shift in Ohio. The role involves assisting with personal care, mobility, hygiene, and feeding under RN supervision, while ensuring safety and comfort for hospice patients.

Qualified applicants will have a state-approved CNA certification, CPR, and a high school diploma or GED; prior hospice/home health experience is preferred. Must be 18+, with a valid driver’s license and reliable vehicle.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification (hands-on demonstration preferred).
• Covid vaccination per facility policy.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understand hospice philosophy and provide terminally ill patient care.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care: baths, hygiene, bedding, grooming as assigned.
• Assist with dressing, feeding, and mobility, reporting needs to RN Case Manager.
• Ensure patient safety using equipment and safe transfer techniques.
• Maintain confidentiality and communicate effectively with patient and family.
• Observe and report changes in patient condition and pain levels to RN Case Manager.
