Job summary

Affinity Health Management in Parma, OH is seeking a Hospice Aide to work the night shift. You will assist with personal care, bathing, dressing, and feeding under the supervision of an RN and follow the patient’s plan of care.

You will report observations, document care in EMR, help with transfers and mobility, and communicate compassionately with patients and families while maintaining safety and quality of care.

Qualifications

• Completion of a state-approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• COVID vaccination as per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.

Responsibilities

• Provide personal care including baths, grooming, and linen changes.
• Assist with feeding and ambulation as directed by the RN.
• Report observations and changes to the RN Case Manager promptly.
• Maintain patient safety and use equipment properly.
• Document care provided in EMR accurately.
• Support family with compassionate communication and education.
