Affinity Hospice Management LLC is seeking a dedicated Hospice Aide for night shifts (5pm - 10pm) in Cleveland, OH. The role reports to an RN Case Manager and delivers personal care, observation, and comfort measures per the patient’s plan of care.

Responsibilities include safety-focused assistance, documentation in EMR, and collaboration with patients, families, and care teams to ensure compassionate end-of-life care.

Qualifikationen

• Completion of a state approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification.
• COVID vaccination per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Comfort with hospice philosophy and terminally ill care.
• At least 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• Licensed driver with insured vehicle in good working order.

Aufgaben

• Perform personal care tasks and respond promptly to patient needs.
• Communicate with patient and family in a respectful, confidential manner.
• Ensure safety and proper use of equipment during care.
• Provide bathing, oral hygiene, grooming, and linen changes as assigned.
• Assist in dressing, feeding, and transfers as directed.
• Support with self-administered medications as allowed by state rules.
• Maintain clean living area and assist with light homemaker tasks.
• Document observations and report changes to RN Case Manager.
• Adhere to care procedures and participate in quality improvement activities.
