Affinity Health Management seeks a Hospice Aide for night shifts to assist with personal care, feeding, and comfort for hospice patients under RN supervision. You will report observations, document care, and promote safety and dignity in a home-like setting.

The role requires CNA certification, CPR, and a HS diploma or GED, with prior hospice or home health experience preferred. Covid vaccination is required per policy. 5pm–10pm shift, on-site support at patient locations.

Formación

• Must have completed state-approved certified nursing assistant course or equivalent.
• Current Healthcare Provider CPR certification.
• Covid vaccination required due to patient population and facility policy.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Comfort with hospice philosophy and terminally ill patients.
• 1 year hospice/home health/long-term care experience preferred.
• Basic computer skills to document in EMR.

Responsabilidades

• Provide personal care including baths, grooming, and basic hygiene.
• Assist in feeding and mobility, and help with transfers as directed.
• Report any pain, changes, or concerns to the RN Case Manager.
• Maintain patient safety and proper use of equipment.
• Document care and observations in EMR per procedures.
• Help with light housekeeping and linen changes as needed.
• Provide respite support to family/caregivers when appropriate.
• Adhere to care plans, safety, and confidentiality policies.
