Affinity Hospice Management LLC is seeking a hospice aide for the night shift (5pm - 10pm) to support patient care under RN supervision. You will perform personal care tasks, assist with activities of daily living, and document observations in the EMR.

The role emphasizes safety, confidentiality, and compassionate communication with patients and families. The position requires a CNA course completion, CPR certification, ability to read and follow instructions, and prior hospice/long-term care

Qualifications

• Completion of a state approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Must be covid vaccinated due to multiple facilities and guidelines.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care given.
• Experience in hospice or long-term care is preferred.
• Basic computer skills to document in EMR.

Responsabilités

• Perform personal care tasks after assignment and attend to patient requests promptly.
• Communicate with patient and family in a respectful, confidential manner.
• Ensure safety and use equipment properly (foot stools, side rails, O2).
• Provide baths, back rubs, oral hygiene, shampooing and changing bed linens.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding and avoid serving homemade foods prepared by the aide.
• Monitor skin condition to prevent tissue breakdown over bony prominences.
• Report pain or changes in condition to the RN Case Manager.
• Assist in ambulation and prescribed exercises.
• Provide ROM and transfers/positioning as directed.
• Assist with self-administered medications as allowed by state regulations.
• Offer bedpans/urinals and assist with light laundry and housekeeping tasks.
• Document care performed and maintain departmental standards.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and needs.
