Job summary

Affinity Health Management is seeking a hospice aide to work under the supervision of a registered nurse on the night shift. The role involves providing personal care, monitoring patient comfort, and reporting observations to the RN Case Manager.

Emphasis is on compassionate communication, safety, and maintaining patient dignity in care plans. The aide assists with daily activities, mobility, and basic tasks while adhering to care procedures and confidentiality.

Qualifications

• CNA certification or state-approved CNA course.
• CPR certification is required (not internet-based).
• Covid vaccination as per facility policy.

Responsibilities

• Perform personal care and assist with daily living activities.
• Report observations and care to the RN Case Manager.
• Provide patient and family with compassionate communication and confidentiality.
• Assist with dressing, grooming, feeding, and mobility.
• Monitor skin and prevent tissue breakdown; maintain safety with equipment.
