Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to work under the supervision of an RN. Responsibilities include assisting with personal care, meals, mobility, and documentation in EMR to support quality, continuity, and safety of patient care.

Ideal candidates have a state-approved CNA or NAC, CPR certification, and a high school diploma or GED, with prior hospice or home health exposure preferred. Must be at least 18 years old and possess a valid driver’s license.

Qualifications

• Completion of a state-approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification (hands-on skills observed).
• Current Food Handlers Permit.
• High school diploma or GED.

Responsibilities

• Provide personal care including baths, dressing, and grooming.
• Report changes in patient condition to the RN Case Manager.
• Assist with mobility, transfers, and range of motion.
• Support patient and family with communication and confidentiality.
• Assist with meals and light homemaker tasks as assigned.
• Maintain a clean and safe patient living area.
• Document care provided in EMR as required.
• Adhere to Dept. of Health reporting requirements when applicable.
