Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide for the night shift from 5pm to 10pm. You will provide essential personal care, support family communication, and document observations in the EMR under RN supervision.

The role emphasizes patient comfort, safety, and adherence to care plans, with responsibilities including bathing, mobility assistance, and light housekeeping as needed.

Qualifications

• Completion of a state approved CNA course.
• Current Healthcare Provider CPR certification.
• COVID vaccination per policy and facility guidelines.
• High school diploma or GED required.
• Ability to read and follow written instructions.
• Hospice or long-term care experience preferred.
• Minimum age 18 and valid driver's license when applicable.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family with respect and confidentiality.
• Ensure safety and proper use of equipment (stools, rails, O2).
• Perform baths, oral care, hair care, linen changes as assigned.
• Assist in dressing, feeding, and mobility as directed by RN.
• Document observations and care in the EMR as required.
• Report pain, changes in condition, or safety concerns to RN Case Manager.
• Maintain clean living area and assist with light homemaker tasks.
