Affinity Hospice Management LLC is seeking a Night Shift Hospice Aide from 5pm to 10pm to work under the supervision of a registered nurse. You will perform personal care, assist with activities of daily living, and report observations to the RN Case Manager.

The role emphasizes comfort, safety, and compassionate communication with patients and families, with responsibilities including safety, documentation in EMR, and participation in quality improvement activities.

Qualifications

• State-approved CNA course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Covid vaccination required due to facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and comfort with terminally ill patients.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsabilités

• Perform personal care and respond promptly to patient requests.
• Communicate with patient and family in a respectful, confidential manner.
• Ensure safety and use equipment properly.
• Provide baths, grooming, oral care, shampoos, and linen changes.
• Assist with dressing and undressing.
• Assist with feeding as assigned; avoid homemade foods.
• Monitor skin and prevent tissue breakdown.
• Report changes to the RN Case Manager.
• Assist in ambulation and prescribed exercises.
• Provide ROM and transfers as directed.
• Assist with medications reminders as allowed by state regulations.
• Assist with bedpans/urinals and light housekeeping.
• Maintain clean living area and document care.
• Participate in quality improvement teams.
• Respect patient and family environment.
• Report violations and abuse per applicable regulations.
• Report suspected abuse/neglect to authorities.
