Job summary

Volunteers of America Los Angeles is seeking an Entry Case Manager to provide intake and service coordination for residents in interim and permanent housing. You work on a team delivering assessment, care planning, coaching, and system navigation to connect clients with community resources.

This on-site role emphasizes advocacy, outreach, and capacity building to support individuals who were experiencing homelessness and are now mostly housed.

Qualifications

• Must be able to pass a fingerprint clearance and background checks.
• Ability to perform under emotional stress when assisting vulnerable clients.
• Experience in case management or related social services is preferred.

Responsibilities

• Assessment and team-based planning to identify client strengths and needs.
• Coaching and social support to connect participants with services.
• Care coordination and housing navigation across programs.
• Advocacy for participants with external agencies and providers.
• Capacity building to motivate progress and engagement.
• On-site outreach and engagement to enroll clients in programs.
