Job summary

Volunteers of America Los Angeles is seeking an Entry-level Case Manager to work on-site with clients in interim and permanent housing. You will coordinate assessments, care planning, coaching, advocacy, and service connections to support individuals and families facing homelessness.

The role emphasizes collaboration within a team, on-site duties, and capacity building to help clients access health and social services and achieve stable housing outcomes.

Qualifications

• Fingerprint clearance and background check required.
• Ability to work under emotional stress with challenging clients.
• 6 Months of Social Services case-related experience or an Associate’s Degree in lieu of experience.

Responsibilities

• Assess participant strengths and needs as part of a team.
• Provide coaching and social support to participants.
• Coordinate care, case management, and housing navigation services.
• Advocate for participants before clinical, agency and community settings.
• Build participant capacity and connect to appropriate programs.
• Outreach and engagement to locate participants and connect them to services.
