Job summary

Volunteers of America Los Angeles is seeking an Entry-level Case Manager to provide coordinated case management for individuals and families in interim and permanent housing. You will assess, plan, coach, and connect clients to health and social services, helping them navigate housing and community resources.

Responsibilities include coordination of care, advocacy, outreach, and capacity building, while maintaining on-site service delivery and alignment with program goals.

Qualifications

• Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications.
• Ability to perform under circumstances of possible emotional stress and conflicts.
• Case Management training within 6 months of hire.

Responsibilities

• Assessment – Work as part of a team to assess participant strengths and needs.
• Coaching for social support of participants.
• Care coordination, case management, system and housing navigation.
• Advocacy for participants before other agencies or community settings.
• Capacity building – motivate participants and connect to appropriate programs.
• Outreach and engagement – locate participants and connect to services.
