Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to coordinate services for low-acuity clients on-site. You will conduct assessments, develop action plans, and connect clients with resources to meet various needs.

Qualifications

• Minimum 1 year in case management or similar social services role.
• Bachelor’s Degree in Social Services-related field may substitute for experience.
• Fingerprint clearance and background checks are required.

Responsibilities

• Conduct comprehensive screenings and assessments to collect functional, environmental, psych-social, financial, employment, housing, education and health information.
• Develop case plans addressing barriers and coordinate services to meet needs.
• Coordinate case conferences, monitor progress, and advocate for clients before agencies.
