Job summary

Habitat Health is seeking an Enrollment RN Per-Diem to lead the clinical enrollment process for the PACE program, coordinating with Outreach and Enrollment Specialists and the IDT to assess eligibility and conduct home safety assessments.

The role involves educating prospective participants and caregivers, ensuring compliant documentation in the EHR, and supporting informed enrollment decisions while traveling to participants' homes within the service area.

Qualifications

• Associate's degree in Nursing; RN license in state.
• Minimum of one year nursing experience with older adults or complex medical needs.
• Ability to conduct home safety and clinical assessments.
• Strong organizational and communication skills.
• Willingness to travel within the service area.
• Valid driver's license and auto insurance.

Responsibilities

• Partners with Outreach Specialist, Enrollment Specialist, and IDT to support enrollment in a coordinated, participant-centered way.
• Conducts nursing, home safety, and eligibility assessments in participants' homes and care settings.
• Performs NFLOC assessments and documents in accordance with regulatory requirements.
• Participates in enrollment eligibility meetings, presenting clinical findings.
• Builds trust with participants and caregivers by education and responsive enrollment experiences.
• Maintains accurate clinical documentation in the EHR and related systems.
