Job summary

Element Care PACE seeks an Enrollment Coordinator to prioritize and guide the PACE enrollment process for prospective participants. You will liaise between participants, caregivers, the enrollment team, IDT, and referral sources to ensure timely coordination.

The role requires in-office work with field visits, Monday–Friday, 8am–4pm, and collaboration with multiple departments to secure required documents and drive census goals.

Qualifications

• Associates required; bachelor’s degree in social work, sociology, psychology, business, public health, or a related field preferred.
• Minimum one (1) year exp. working with seniors and their caregivers in a healthcare and/or mental health setting.
• Advanced computer skills, including Microsoft Office and Customer Relationship Management software.
• Ability to communicate effectively and build trust with licensed medical personnel.
• Spanish speaking preferred.

Responsibilities

• Explain the PACE model to prospective enrollees and caregivers.
• Ensure eligibility criteria are met per enrollment regulations.
• Communicate with enrollment team, IDT, and referral sources about progress.
• Obtain and submit financial documents from prospective participants.
