Job summary

Habitat Health is seeking an Intake Coordinator to facilitate the enrollment process for prospective participants in our PACE program. You will coordinate home visits, center tours, assessments, and follow-ups, partnering with outreach, clinical pre-enrollment, and care teams to ensure a smooth, participant-centered experience.

Responsibilities include maintaining accurate participant information in CRM/EMR, scheduling, documentation, and educating families about the PACE model.

Qualifications

• Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or related field.
• At least 1 year of healthcare enrollment or participant services experience.
• Ability to coordinate multiple priorities with attention to detail.
• Experience with Medicare/Medicaid or eligibility programs preferred.
• Strong communication and collaboration skills with cross-functional teams.
• Ability to work independently while collaborating with others.
• Proficiency in Microsoft Office and quick learner of CRM/EMR systems.
• Valid driver's license and ability to travel within the service area.
• Active CPR/BLS certification or ability to obtain within 30 days.
• Meets health requirements including TB and immunizations.

Responsibilities

• Coordinate end-to-end intake for prospective participants with cross-functional teams to support a seamless enrollment experience.
• Schedule home visits, center tours, assessments, and follow-ups to progress through intake.
• Maintain proactive communication with Outreach & Enrollment and clinical teams for updates and coordination.
• Partner with clinical teams to identify and coordinate additional assessments for eligibility and IDT presentations.
• Deliver a welcoming, participant-centered experience and educate about the PACE model and Habitat Health services.
• Maintain accurate participant information, referral activity, and intake documentation in CRM/EMR and other systems.
• Coordinate and track participant requirements through each stage of intake and enrollment to ensure timely completion of documentation and activities.
• Support high-quality handoffs from intake through enrollment and into the IDT.
• Assist with community events and enrollment conferences and other initiatives to promote awareness.
