Job summary

Element Care seeks an Enrollment Coordinator to prioritize and organize the PACE enrollment process for prospective participants and caregivers. You will serve as the executive liaison among participants, caregivers, the enrollment team, IDT, and referral sources to ensure timely coordination.

You will conduct visits, verify eligibility, and help gather required documents, both in-office and in-home as needed, with a focus on achieving census goals.

Qualifications

• Associates required; Bachelor’s preferred in related fields.
• Minimum one year experience with seniors in healthcare/mental health.
• Advanced computer skills: MS Office and CRM.
• Strong communication and trust-building with medical personnel.

Responsibilities

• Explain the PACE model to prospective enrollees and caregivers.
• Verify eligibility according to enrollment regulations.
• Evaluate health status using a strengths-based approach.
• Communicate enrollment status with the team and referral sources.
• Coordinate financial documents for enrollment submissions.
• Attend Enrollment Team meetings to address referrals and barriers.
• Arrange visits/tours of PACE Health Centers and coordinate IDT meetings.
• Complete enrollment steps and orient new enrollees to sites and documents.
