Job summary

High Desert PACE Inc. seeks an Enrollment RN to determine eligibility for its PACE program. You will work with physicians, social workers, and enrollment staff to assess participants in homes or centers and prepare state review documentation.

The role requires an RN license in California, an associate degree, CPR, and 3 years of geriatric healthcare experience, with preference for frail elderly experience. Use of an EHR system and cross-team coordination are essential.

Qualifications

• Associate degree from accredited nursing program.
• Current Registered Nurse license in California.
• CPR certification or ability to obtain.
• 3 years healthcare experience with geriatrics emphasis.
• 1 year experience with frail elderly preferred.

Responsibilities

• Determine eligibility for High Desert PACE participants.
• Conduct home visits and center-based assessments.
• Collaborate with IDT and enrollment teams.
• Prepare documentation for state review timely.
• Coordinate with DHCS nurse assessors and LOC reviews.
• Maintain confidentiality and privacy of participant data.
