Job summary

Habitat Health is seeking an RN Care Manager to deliver personalized, longitudinal nursing care to a panel of PACE participants, conducting assessments in both center and home settings and coordinating with the care team to implement care plans.

You will lead IDT meetings, provide education to participants and caregivers, and manage transitions of care, including complex discharges. A California RN license and case management experience are required.

Qualifications

• Graduate of an accredited school of nursing or equivalent experience.
• Unencumbered California Registered Nurse (RN) license required.
• Minimum 2-4 years of experience clinically caring for medically complex or older adults’ population as an RN.
• Minimum 1-2 year's experience in case management.
• Strong clinical acumen in chronic disease management and complex geriatric care.
• Experience in management of clinical interventions: wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.
• CPR/BLS certification required or obtained within 30 days of employment.
• Experience with frail or elderly population.
• Ability to thrive in a fast-paced, evolving environment.

Responsibilities

• Contribute to a center experience that Participants want to spend time in and foster belonging.
• Participate in Interdisciplinary Team (IDT) meetings with insights from assessments and care plans.
• Conduct face-to-face nursing assessments across physical, psychosocial, and behavioral domains.
• Deliver personalized care for a panel of participants based on care plans.
• Document nursing interventions and respond to physician orders promptly.
• Coordinate care transitions and complex discharges with hospital and long-term care facilities.
• Coordinate medications, equipment, referrals, and supplies.
• Educate participants and caregivers on care plans.
• Perform wound care, IV therapies, and related procedures within RN scope.
