Job summary

Trinity Health PACE is seeking an experienced Registered Nurse to oversee home-based service delivery for program participants. You will provide skilled nursing care, prescribe treatments, and develop care plans in line with professional standards and primary care orders.

The role includes assessments, coordination with an interdisciplinary team, counseling participants and families, and supporting integration of new participants into the PACE program.

Qualifications

• Graduate of an accredited Nursing Degree program.
• Current, unencumbered RN license in Maryland
• CPR Certification required.
• Minimum two years of experience with geriatric population.
• Drivers license, insurance and reliable transportation throughout our service area.
• Proficiency in Microsoft Office.
• Ability to perform standard clinical procedures and demonstrate solid problem-solving skills.
• Superior written and verbal communication skills.
• Ability to change and be flexible with work priorities.

Responsibilities

• Provide skilled nursing care and prescribed treatments.
• Participate in interdisciplinary team meetings.
• Facilitate integration of new participants.
• Communicate and counsel participants and families.
• Complete participant nutritional assessments and reassessments as needed
• Participate as a member of the interdisciplinary team.
• Facilitate integration of new participants into the PACE program.
• Communicate, counsels and guides participants and families regarding service needs.
