Job summary

Family Health Centers of San Diego seeks a Registered Nurse for PACE Enrollment to assess older adults for safety, eligibility, and independence in the community. You will evaluate home environments and determine eligibility for Level of Care assessments using established criteria and guidelines.

The role requires CA RN license, 1–2 years with older adults, and travel between sites. Bilingual English/Spanish preferred; benefits include competitive salary and comprehensive coverage.

Qualifications

• California State Board RN License required.
• 1–2 years of experience working with older adult population.
• Current American Heart Association CPR (BLS) or ACLS/ATLS.
• Graduation from a recognized RN/BSN program.
• Willingness to travel between sites; valid CA driver's license and proof of insurance.

Responsibilities

• Assess prospective participants for PACE eligibility and safety of home environment.
• Complete DHCS LOC and Certification forms for enrollment approval.
• Develop and review home care plans of care with participants and families.
• Collaborate with RN manager to ensure compliance and quarterly audits.
