Job summary

Habitat Health is seeking an Enrollment RN Per-Diem to lead clinical aspects of the enrollment process for our PACE program in California. You will evaluate eligibility, perform home safety assessments, and educate prospective participants and caregivers.

In partnership with the Outreach Specialist and IDT, you will ensure a welcoming, compliant enrollment experience while maintaining meticulous clinical documentation and participating in continuous improvement efforts across enrollment

Qualifications

• Associate's degree in Nursing from an accredited program.
• Active, unrestricted RN license in the applicable state.
• At least 1 year of nursing experience with older adults or complex medical needs.
• Ability to conduct clinical and home safety assessments with sound judgment.
• Strong organizational and communication skills for cross-functional teams.

Responsibilities

• Serve as clinical lead during enrollment, coordinating with IDT and outreach teams.
• Perform nursing, home safety, and eligibility assessments in homes and care settings.
• Complete NFLOC assessments and related documentation per DHCS requirements.
• Present clinical findings at enrollment eligibility meetings and support informed decisions.
• Maintain accurate EHR documentation and stay current with regulatory requirements.
