Job summary

Valir PACE is seeking a PRN RN to provide flexible clinical support as an RN Care Coordinator and on-call nurse for our PACE program. You will deliver comprehensive nursing care, assessments, and coordination for participants and prospective enrollees, collaborating with the IDT to create individualized care plans that promote safety and quality of life.

Qualifications

• Current RN license in good standing (state-specific).
• Current CPR Certification.
• Valid Driver’s License (for field-based work).
• ACLS and UCAT certifications preferred (as applicable).
• Experience in geriatric, home health, or PACE preferred.
• Prior experience with in-home assessments and care coordination strongly preferred.

Responsibilities

• Provide flexible clinical support to the PACE program as RN Care Coordinator (RNCC) and on-call nurse.
• Deliver comprehensive nursing care, care coordination, and participant assessments for PACE participants and enrollees.
• Collaborate with the IDT to develop and implement individualized plans of care.
• Perform skilled nursing services in the PACE center and participant homes.
• Coordinate urgent assessments, medications management, chronic disease monitoring, and care transitions.
• Educate participants and caregivers; support aging in place while preventing hospitalizations.

Education

RN license (state-specific)

CPR Certification

Driver’s License
