Job summary

Habitat Health is seeking an Intake Coordinator to streamline the transition of prospective participants into our PACE program. You will coordinate home visits, tours, assessments, and enrollment steps in conjunction with outreach, clinical teams, and caregivers.

You will manage participant data in CRM/EMR, ensure timely documentation, and educate families about Habitat Health services and the PACE model. This role requires strong communication and organizational skills in a fast-paced setting.

Qualifications

• Bachelor’s degree in Healthcare Administration, Business Administration, Marketing, or related field, or equivalent experience.
• Minimum 1 year in healthcare, enrollment, participant services, care coordination, customer service, or similar role.
• Strong attention to detail with the ability to manage multiple priorities and timelines.
• Experience with Medicare/Medicaid or enrollment processes preferred.
• Excellent communication and collaboration across teams to support intake and enrollment.

Responsibilities

• Coordinate end-to-end intake process with cross-functional teams for timely enrollment.
• Arrange home visits, center tours, assessments, and follow-up appointments.
• Maintain proactive communication with Outreach, Enrollment, and clinical pre-enrollment teams.
• Coordinate additional assessments to support eligibility and IDT presentations.
• Educate prospective participants about the PACE model and Habitat Health services.
• Maintain accurate participant data in CRM/EMR and other systems.
• Track requirements through intake to enrollment, ensuring timely documentation.
• Support smooth handoffs from intake to enrollment and IDT, and assist with events and outreach.
• Comply with federal/state regulations and Habitat Health policies.
