Job summary

Socket.dev is seeking a Transitional Housing Case Manager to provide intensive, trauma‑informed case management and supportive services to unaccompanied female victims of crime or trauma experiencing homelessness. The role emphasizes building safe, trusting relationships, clear boundaries, and helping participants pursue stable housing and growth.

You will collaborate with landlords, housing authorities, and community agencies; participate in MDT meetings; develop measurable service plans;

Qualifications

• Bachelor’s degree in social work or related field; Master’s preferred.
• Experience with vulnerable/at-risk populations and homelessness.
• Knowledge of community resources and housing programs.

Responsibilities

• Plan for discharge and participate in MDT meetings to review goals.
• Provide intensive, trauma‑informed case management and referrals.
• Assist housing search and placement with landlords and agencies.
• Develop service plans, safety plans, and coordinate move‑in within timelines.
• Coordinate with community providers and document incidents as needed.
