Job summary

Socket.dev is seeking a Case Manager to support residents by setting clear, measurable goals toward housing permanency and coordinating services such as healthcare, job training, housing, and immigration resources. Build collaborative relationships with clients based on empathy, accountability, and trust.

You will interview new admissions within 24 hours, develop individualized plans with short- and long-term goals, and review progress bi-weekly.

Qualifications

• Bachelor’s degree in human services or related field.
• Experience with homeless populations is preferred but not required.
• Strong written and verbal communication skills.
• Ability to work under pressure and handle crises.
• Bilingual in Spanish, French or Haitian Creole is a plus.

Responsibilities

• Maintain a caseload of 25 clients.
• Interview all new admissions within 24 hours.
• Conduct intakes and assess residents’ needs; coordinate referrals.
• Develop individualized service plans with goals; review bi-weekly.
• Monitor progress through weekly appointments; document outcomes.
• Maintain organized case files and ensure timely updates.
• Collaborate with shelter staff and service providers to remove barriers.
• Advocate for clients in interactions with external agencies.
• Transport clients to appointments when needed; assist with housing re housing.
• Engage in community coalitions and ongoing trainings as assigned.
