Commonwealth Healthcare at Home, a part of the LHC Group and Optum family, is seeking a Physical Therapist Assistant to carry out treatment plans and educate patients in exercise and mobility within home care. The role emphasizes working under a supervising PT and coordinating with the broader care team.

Prerequisites include licensure to assist in physical therapy, a valid driver’s license, and weekend availability; CPR certification is preferred.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsabilités

• Follow state laws governing home care physical therapy; perform only supervised procedures and suggest plan-of-care changes.
• Instructs patients in active/passive exercise, gait, ADL, transfer, safety, and prosthetic training.
• Observe and report patient responses to treatment; coordinate care with the health care team.
• Educate patients, family, and caregivers on therapy within the scope of the PTA.
