Qualifications

• Bachelor's or master's degree in one of the listed concentrations (Psychology, Sociology, Social Work, Nursing, Counseling, Human Services Administration).
• Valid Driver's License and an insured vehicle.

Responsibilities

• Provide targeted case management services to individuals with disabilities or medical needs.
• Establish and monitor Person Centered Plans of Service (POS) with outcomes and required supports.
• Coordinate and link services, resources, and referrals to achieve POS goals.
