Job summary

Capital Health seeks a Pharmacy Technician to obtain medication histories from ED patients, document data into the EHR for pharmacist review, and support inpatient tech tasks as needed.

The role requires a CPhT, NJ pharmacy registration (or plan to obtain within 6 months), and at least two years of tech experience. Strong communication and attention to detail are essential in a busy ED setting.

Qualifications

• High school diploma or equivalent.
• Pharmacy Technician Certification Board (CPhT) certification required.
• NJ Board of Pharmacy registration or acquire within 6 months of employment.
• Two years of Pharmacy Technician experience required.
• PTCB Medication History certification preferred.

Responsibilities

• Conduct detailed interviews with patients/families to compile current home medications (name, dose, route, frequency, indication, last dose).
• Prompt for additional medications including OTCs and herbal products.
• Contact outpatient pharmacies and providers to verify regimens and discrepancies.
• Obtain allergy history and reaction details.
• Utilize EMR/EHR systems to review and document medication history.
• Record medication and allergy data accurately in the EHR for pharmacist review.
• Participate in performance improvement activities and LeapFrog data collection.
• Precept pharmacy technicians, students, and residents in home medication interviews and documentation.
• Perform other duties as assigned.
