Capital Health in Trenton, NJ seeks an experienced Pharmacy Technician to join our ED-focused pharmacy team. You will obtain medication histories for new ED patients and document them in the electronic health record, supporting pharmacists and clinicians in patient care.

Requirements include a High School diploma, CPhT certification, NJ registration within 6 months, and two years of pharmacy tech experience. Strong communication and accuracy with EHRs are essential for success.

Qualifikationen

• Education: High school diploma or equivalency and CPhT certification required.
• Current NJ Board of Pharmacy Registration or obtain within 6 months of employment.
• Two years Pharmacy Technician experience required; PTCB Medication History preferred.

Aufgaben

• Conducts detailed interviews with patients and/or caregivers to compile medications list with dose, route, frequency and indications.
• Prompts for OTC meds, herbals, vitamins, patches and other products to complete home med history.
• Contacts outpatient pharmacies and providers to verify regimens and resolve discrepancies.
• Obtains allergy history and records reactions and severity.
• Uses EHR databases to document medication histories for pharmacist review.
• Records information accurately into the EHR for pharmacist and clinician review.
• Precepts pharmacy staff and students on home medication interviews and documentation.
• Participates in data collection related to medication reconciliation.
• Able to perform inpatient pharmacy technician tasks as needed.
• Performs other duties as assigned.
