Job summary

Bon Secours is seeking a Social Worker Care Manager at Southside Regional Medical Center to coordinate discharge planning and transitions for patients and families across the continuum of care. You will access hospital, community, and governmental resources and provide supervision to peers and students.

The role requires a Bachelor of Social Work, a Master’s degree preferred, BLS, and state licensure with ACM/ CCM preferred. Experience in clinical settings and care coordination is valued.

Qualifications

• Bachelor of Social Work is required.
• Master’s degree in social work or healthcare related field is preferred.

Responsibilities

• Identifies and prioritizes patients in need of social services, using a biopsychosocial approach and plans with patient and care team.
• Completes all necessary documentation in patient records with clear, concise entries.
• Documents plan of care addressing post-hospital needs and ensures patient/family involvement.
• Follows standardized practices related to Advance Care Planning and readmission prevention.
• Supports care progression by addressing barriers and participating in discharge rounds.
• Promotes proactive care to remove barriers to timely testing and treatment.
