Job summary

Mercy Health is seeking a Social Worker Care Manager (PRN) for St. Francis Eastside. The role focuses on discharge planning and coordinating services to ensure smooth transitions across care settings.

You will provide clinical supervision to peers and students, collaborate with the healthcare team, and address barriers to timely care, while adhering to established practices for Advance Care Planning and readmission prevention.

Qualifications

• Bachelor of Social Work (required).
• Master's degree in social work or healthcare related field (preferred).
• 1 year of experience in clinical setting (required).
• 3 years of experience in an acute care clinical setting (preferred).
• Ambulatory or post-acute care coordination experience (preferred).

Responsibilities

• Identifies and prioritizes patients in need of social services, coordinating with patient, caregivers and healthcare team.
• Completes all necessary documentation with clear, concise patient records.
• Ensures transition of care planning reflects patient and caregiver goals and engages multidisciplinary team.
• Follow standardized practices related to Advance Care Planning, Length of Stay management and readmission prevention.
• Supports denial prevention by addressing barriers to progression of care and participates in discharge rounds.
• Promotes proactive care to remove barriers to timely testing and treatment while using resources efficiently.
