Job summary

Bon Secours is seeking a Social Worker Care Manager for the St. Francis Downtown location. This weekend-only role focuses on discharge planning, patient transition, and coordination with hospital and community resources.

The position includes clinical supervision responsibilities for peers and students. Requirements include a Bachelor of Social Work and state licensure. A Master’s degree is preferred; BLS is required, along with strong communication, advocacy, and collaboration skills to support

Qualifications

• Bachelor of Social Work is required.
• Master’s degree in social work or healthcare related field preferred.
• Licensed Social Worker in state of practice is required or preferred.

Responsibilities

• Identifies and prioritizes patients in need of social services using a biopsychosocial approach.
• Documents thoroughly in the patient record to reflect needs and plans.
• Coordinates discharge planning and transition of care with the care team.
• Supports Advance Care Planning and readmission prevention using standardized practices.
• Provides clinical supervision to peers, social workers, and students.
• Upholds patient advocacy and ethics in all interactions.
