Job summary

Bon Secours is seeking a Social Worker Care Manager for St. Francis Downtown (weekends only, up to 24 hours). The role focuses on discharge planning, patient advocacy, and coordinating resources to ensure smooth transitions across care settings.

The ideal candidate holds a BSW, with MSW preferred, and is licensed/eligible to practice; experience in acute care and post-acute care coordination is valued.

Qualifications

• Bachelor of Social Work required; MSW preferred.
• LCSW/ LICSW status may be preferred in some markets.
• Experience with discharge planning and care coordination.

Responsibilities

• Provide interventions and discharge planning to patients and families.
• Facilitate smooth transitions across continuum of care with hospital, community and governmental resources.
• Provide clinical supervision to peers, social workers, and students.
• Identify and prioritize patients needing social services with a holistic approach.
• Complete and maintain clear, timely documentation in the patient record.
• Support care planning and communications to the multidisciplinary team.
