Qualifications

• Bachelor of Social Work is required.

• Licensed as a Social Worker in the state of practice is required, with VA preference.
• BLS Basic Life Support is required.
• ACM or CCM certification preferred.

Responsibilities

• Identify and prioritize patients in need of social services within a biopsychosocial framework.
• Complete documentation in the patient record with clear, timely notes.
• Coordinate discharge planning and transitions across care settings.
• Provide clinical supervision to peers, social workers, and students.
• Address barriers to progression of care and participate in discharge rounds.
