The position requires a Bachelor of Social Work, and a Master’s in a related field is preferred; licensure in VA is required or preferred. Clinical supervision of peers and students is included as part of the role.

Qualifications

• Bachelor of Social Work (required)
• Master’s degree in social work or healthcare field (preferred)
• Licensed as a Social Worker (VA preferred)
• 1 year of experience in clinical setting (required)
• Ambulatory or post-acute care coordination experience (preferred)

Responsabilités

• Identify and prioritize patients in need of social services using a biopsychosocial approach.
• Document plans of care clearly in the patient record.
• Coordinate discharge planning and transitions across hospital and community resources.
• Follow standardized practices related to Advance Care Planning and Readmission prevention.
• Provide clinical supervision to peers, social workers, and students.
