Bon Secours Mercy Health in Ohio seeks a Social Worker Care Manager to provide discharge planning and intervention services, ensuring smooth transitions across the continuum of care by leveraging hospital, community, and governmental resources.

The role includes supervising peers and students, documenting plans, and promoting timely care while aligning with patient goals and preferences in collaboration with the care team.

Qualifications

• Bachelor of Social Work is required.
• Master’s degree in social work or healthcare-related field preferred.
• BLS Basic Life Support and state licensure as a Social Worker are required (VA preferred).

Responsibilities

• Identify and prioritize patients in need of social services using biopsychosocial, functional, cultural, spiritual, and financial factors.
• Document care plans clearly in the patient record to reflect patient goals and caregiver involvement.
• Coordinate discharge planning across hospital, community resources and government entities.
• Participate in Interdisciplinary Discharge Rounds and advance care planning.
• Support and promote timely testing and treatment by addressing barriers to progression of care.
