Job summary

Catholic Health Services is seeking a Social Worker to coordinate discharge planning for residents, addressing psychosocial needs and acting as a liaison between residents, physicians, and nurses. You will help residents access community resources, conduct psychosocial histories, educate on Resident Rights and advance directives, and participate in Care Plan meetings.

Requirements include MSW/BSW or related degree, 1 year supervised healthcare social work, CPR certification, computer/EMR

Qualifications

• MSW/BSW or related degree required.
• 1 year supervised healthcare social work experience required.
• CPR certification required.
• Proficiency with computer office/clinical software (EMR).
• Excellent reading/writing/communication in English.

Responsibilities

• Coordinate discharge planning and act as liaison between residents and physicians/nurses.
• Interviews admissions to complete psychosocial history and verify demographics.
• Educate residents/families on Resident Rights, Advance Directives, resources, and insurance.
• Attend Care Plan meetings with interdisciplinary team; participate in family conferences.
