Job summary

Bon Secours is seeking a Social Worker Care Manager for the St. Francis Eastside location. The PRN role focuses on discharge planning, coordinating hospital, community, and government resources to ensure smooth transitions for patients and families.

The incumbent provides clinical supervision to peers, Social Workers, and students, while prioritizing compassionate care and adherence to Bon Secours values. A BSW is required; an MSW is preferred, with licensure in the state of practice.

Qualifications

• Bachelor of Social Work (required).
• Master's degree in social work or healthcare related field (preferred).

Responsibilities

• Identifies and prioritizes patients in need of social services, using a holistic approach including biopsychosocial, functional, cultural, spiritual, and financial factors.
• Plans with the patient, caregivers and healthcare team to maximize outcomes and revises the plan as patient condition changes.
• Documentation is clear, concise and timely in the patient record reflecting patient needs and transition plans.
• Follow standardized practices related to Advance Care Planning, Length of Stay management and readmission prevention.
• Supports denial prevention by addressing barriers to progression of care and participating in discharge rounds.
• Promotes proactive care to remove barriers to timely testing and treatment, ensuring appropriate resource use.
