HCA Houston Healthcare Southeast is seeking a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating community resources for identified patient populations. The BSW will act as a liaison between patients and post-acute care providers and help implement discharge plans across the continuum of care, primarily in Med/Surg units.

Qualificações

• Bachelor's degree in Social Work or related field.
• Licensed Baccalaureate Social Worker (LBSW) required.

Responsabilidades

• Collaborate with RN case managers, nurses, physicians, insurers and staff to identify patient needs.
• Identify and address discharge barriers across financial, emotional, psychosocial, medical and spiritual aspects.
• Perform assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings with care team, patients, families and caregivers.
• Educate patients and caregivers on community resources and post-acute care criteria and coverage.
