Job summary

Memorial Healthcare System in Florida is seeking a Social Worker to provide discharge planning and care coordination within the acute care setting. This role conducts routine discharge assessments, supports physician consults, and coordinates post-acute services to facilitate safe, timely patient transitions.

You will participate in rounds, interview patients and families, identify resources, and arrange services as part of the discharge plan.

Qualifications

• Bachelor's degree in Social Work required.
• Minimum 1 year of professional social work experience.
• Ability to work in a high-stress environment and take appropriate action.

Responsibilities

• Participates in interdisciplinary rounds and case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
• Interviews and assesses patients and/or family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Assists with discharge planning and processes.
• Identifies resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
