Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will implement ISP/IPP plans, promote independence, and assist with daily living activities under supervision, in Pennsylvania.

The role requires adherence to care plans, documentation, and HIPAA-compliant practices, with flexibility to work evenings, weekends, and overnight shifts as needed.

Qualifications

• High school diploma or equivalent required.
• Prior caregiving/home health experience preferred.
• English proficiency required; bilingual a plus.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medications as authorized.
• Implement behavior support strategies and monitor health and safety.
