Job summary

OneWell Health Care in Pennsylvania seeks a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will help with daily living activities, transportation, health monitoring, and documentation, following ISP/IPP plans and regulatory requirements; flexibility to cover evenings, weekends, and overnight shifts.

A compassionate, team-based role with opportunities for training; field-based travel to homes and community

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving or home health preferred.
• Ability to use electronic documentation and EVV systems.
• English proficiency required; bilingual skills are a plus.
• Valid driver’s license may be required.

Responsibilities

• Assist individuals with daily living activities and personal care.
• Support meal preparation, mobility, and medication assistance as allowed.
• Implement behavior support strategies and monitor health and safety.
• Maintain structured routines and document care notes daily.
• Attend trainings and team meetings; participate in shift coverage.
