Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings in Pennsylvania. You will assist with daily living activities, health monitoring, meal prep, mobility, and transportation while following individualized service plans.

This field-based role requires strong documentation skills, adherence to HIPAA and regulations, and flexibility for evenings, weekends, and overnight shifts.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support for individuals with IDD is preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home/community settings.
• Assist with daily living activities, meal prep, mobility, and health monitoring as authorized.
• Implement behavior support strategies and maintain safety and well-being.
• Maintain documentation and participate in trainings and team meetings.
• Support transportation to appointments and community activities.
